 Evaluation form or Feedback

     As you complete each workshop ,we would appreciate it very much if you would fill in the evaluation form.

The evaluation form can be found at the bottom of last page of each of the workshops .It can also be found as  # 7 in the list of workshops. Every time you finish workshop, please go back to  complete the evaluation form for us.

If you click on the picture of speaker which is the small  gray box at the top of page on the left, you will hear each page read aloud for you.

When you completed a  page and ready to go to the next page, click on the little hand that’s pointing at the top of page on the right.
